Cv Popped out!

I had a new CV joint replaced a month ago (at some cost) Yesterday as I put her (76 Kamper) into reverse I lost drive. I looked underneath and saw the drive shaft hanging down from the motor. The connection to the wheel was also loose. I was able to remove the shaft and tow her home (yes I had a set of allen keys)

Before I take her back. Why would the shaft simply fall off? The bolts and shaft look new.

Is it possible the Mech forgot to put on split washers on the bolts?
Is this a common  occurance?

Any hint or advise greatfully accepted.
